Participation Agreement & Liability Waiver
By booking services with Miss Megan / Farm Day Adventures, I acknowledge and agree to the following:
Participation & Activities
I understand that services may include storytelling, crafts, movement, outdoor play, sensory activities, and age-appropriate creative exploration.
Assumption of Risk
I understand that participation in these activities involves normal risks associated with children’s play, including but not limited to minor injuries, slips, or spills. I voluntarily assume responsibility for these risks.
Health & Allergies
I agree to disclose any relevant medical conditions, allergies, sensory sensitivities, or special needs prior to the session.
Limitation of Liability
I release and hold harmless Miss Megan / Farm Day Adventures from any claims, damages, or liabilities arising from participation, except in cases of gross negligence.
Interactive Storytime Supervision (if applicable)
I understand that unless otherwise agreed in writing, parents, guardians, or hosting organizations are responsible for supervising children during interactive storytime. Miss Megan is not responsible for supervision beyond the scope of the agreed service.

Megan will follow up to coordinate date, details, and payment.
Please note: A completed Participation Agreement & Liability Waiver is required before your session. Miss Megan reserves the right to cancel or postpone any booking if this form has not been received prior to your scheduled visit.
